How to use this
Boards don't need a forty-slide AI update. They need one page that answers four questions: where is AI in our business, what is it costing us, what could hurt us, and what changed since you last told us. This template answers all four.
The structure follows the board-reporting elements in COSO's 2026 guidance on internal control over generative AI, adoption, key risk indicators, incidents, and material changes to high-impact use cases, with a spend and commitments section added, because directors govern capital as well as risk. Tiers and definitions reference the companion AI policy template; the counts come straight out of the model and tool inventory register. The three documents are built to work as a set.
Rules: everything in [brackets] is yours. Keep it to one page, the discipline of the page is the point. If a section runs long, the board needs a separate session, not a longer report. And nothing on this page should be a number you can't evidence; a defensible line of sight means every figure traces to the register or the ledger.
